The short version
Clinicians are the single largest source of clients here, so it is worth being precise about scope rather than leaving you to guess.
Fit
Goals met on paper, function well short of where they want it. Needs progressive load and supervision over months — which is a training problem, not a therapy one.
A stumble, a near-miss, or a first fall. Balance, reaction time and hip strength are all trainable, and in-home work lets the training happen in the exact environment where the risk lives.
The patient who will not join a gym and will not do a handout alone. Someone arriving at the door once or twice a week is the difference between advice and adherence.
Also common: adults 40+ wanting to get ahead of bone density and muscle loss, and couples who will only start if they can do it together.
How to refer
A card, this web address, or simply "call Jenelle." Front-desk staff are welcome to pass on the number without anything in writing.
With the patient's permission, pass the details along and Jenelle will make the first contact instead of leaving it to them.
A sentence is enough — "no loaded flexion six weeks," "weight-bearing as tolerated." It will be followed, and questions will come back to you rather than be guessed at.
Every patient starts with a free intro session — a conversation, not a workout — so there is no cost to them in finding out whether it's a fit. If it isn't, Jenelle will say so.

Questions clinicians ask
Yes, if the patient is happy for you to. Closing the loop matters, and a referral that disappears into silence is a referral nobody repeats.
They'll be told so at the free intro and pointed back to you. There's nothing to be gained from starting someone who should still be under care.
Yes, where the treating therapist agrees and sets the boundaries. More often she's picking up after discharge — there's a page about that here.
No. Personal training needs no referral, and nothing here is billed to insurance, so there's no paperwork trail to maintain.
Yes — cards and a one-page sheet for the waiting room, including versions written for physical therapy, chiropractic and pulmonology practices. Ask and they'll be dropped off.
Colorado Springs and immediate surrounds. Homes well outside the city may carry a small travel fee, always quoted to the patient up front.
Call or email directly, or simply hand over the number. If you'd like printed cards for the front desk, say so and they'll be delivered.
